IPAG eMedikation B8, S tatus der Medikation

Der „Status der Medikation“ hat für einen nachfolgend Behandelnden eine wichtige Bedeutung. Mit Hilfe des Status wird für jedes Arzneimittel eines Patienten zum Beispiel ersichtlich durch wen es neu verordnet, gegebenenfalls substituiert oder abgesetzt wurde. Weitere Details zu diesen Änderungen können allenfalls der Medication History entnommen werden.

Der Therapieentscheid führt zum folgenden Status des Arzneimittels:

Status „Verordnet“

Arzneimittel, die aufgrund eines Therapieentscheids eines Behandelnden eingetra- gen werden, erhalten die Statusbezeichnung „Verordnet“. Mit diesem Status wer- den somit alle Arzneimittel bezeichnet, die Gegenstand einer Verordnung eines Arztes oder eines pharmazeutischen Medikationsentscheids eines Apothekers sind.

Eine detaillierte Beschreibung der verschiedenen Stati steht noch aus und wird in Kapitel 9 „Nächste Schritte“ erwähnt. 

Kapitel 9.2
Der Status der Medikation, so wie er in Kapitel 5.1.1 eingeführt wurde, ist sowohl innerhalb der Arbeitsgruppe als auch zwischen den IPAG-Arbeitsgruppen eMedikation und eAustrittsbericht noch nicht abschliessend diskutiert worden. Diese Diskussion sollte angestrebt werden.
